The Portfolio diet: a plant-based pattern built to lower cholesterol

The Portfolio diet is a plant-based eating pattern assembled specifically to lower LDL cholesterol, and a randomised trial found it did so by about 13% over six months — roughly four times the reduction from standard low-saturated-fat advice [s1]. A large observational study later linked closer adherence to the pattern with lower rates of cardiovascular disease [s2], though, as with any diet, the trial evidence is about a risk marker and the outcome evidence is associational rather than randomised.
What the "portfolio" is
The diet bundles four foods with individually recognised cholesterol-lowering effects: plant sterols, soy protein, viscous (soluble) fibres, and nuts [s1]. The idea is that combining several modest effects in one pattern produces a larger reduction than any single food — a portfolio, in the financial sense, rather than a single bet. It contains no drug; the components are ordinary foods eaten in defined amounts.
The randomised trial
The defining trial, published in JAMA in 2011, enrolled 351 participants with high cholesterol from four academic centres across Canada [s1]. They were randomised to one of three groups for six months: a low-saturated-fat therapeutic diet as the control, or the Portfolio diet delivered at one of two counselling intensities — a routine version with two clinic visits over six months, or an intensive version with seven [s1].
In the modified intention-to-treat analysis of 345 participants, LDL cholesterol fell from an overall mean baseline of 171 mg/dL by 13.8% (95% CI, 10.3% to 17.2%) on the intensive Portfolio diet and 13.1% (95% CI, 9.5% to 16.7%) on the routine version [s1]. The control diet lowered LDL by 3.0% (95% CI, −0.1% to 6.1%) [s1]. Both Portfolio arms beat the control significantly, and — notably — the intensive and routine versions did not differ from each other (P = .66) [s1]. More clinic visits bought no extra benefit; the food did the work.
The effect tracked how closely people stuck to it. Among those on a Portfolio diet, the LDL reduction correlated with dietary adherence [s1] — the expected pattern for a real dietary effect, and a reminder that the trial's tidy averages depend on people actually eating the prescribed foods.
From cholesterol to cardiovascular events
A trial can show a diet moves LDL, but not, in six months, whether it prevents heart attacks. For that the evidence is observational. A 2023 analysis in Circulation pooled three long-running U.S. cohorts — 73,924 women in the Nurses' Health Study, 92,346 in the Nurses' Health Study II, and 43,970 men in the Health Professionals Follow-up Study — and scored each participant's diet against the Portfolio pattern over as long as 30 years [s2]. It documented 16,917 cardiovascular events, including 10,666 coronary heart disease cases and 6,473 strokes [s2].
Comparing the highest with the lowest fifth of adherence, a higher Portfolio Diet Score was associated with a lower risk of total cardiovascular disease (pooled hazard ratio, 0.86; 95% CI, 0.81 to 0.92), coronary heart disease (0.86; 95% CI, 0.80 to 0.93) and stroke (0.86; 95% CI, 0.78 to 0.95) [s2]. A roughly 14% lower relative risk is consistent with the trial's cholesterol effect, but it is an association: people who eat this way differ from those who do not in ways a statistical model can only partly adjust for.
A funding note worth making
One detail belongs in the record. The senior authors of the Portfolio diet research have extensive financial ties to the food industry, including research funding and food donations from nut, soy and other commodity groups whose products the diet promotes [s2]. That does not invalidate the findings — the randomised trial's result is a measured outcome, not an opinion — but it is the kind of provenance a reader should be told about rather than left to discover, particularly for a diet defined by specific food categories.
Where it fits
The Portfolio diet occupies a specific niche: a pattern engineered for one job, lowering LDL, with a randomised trial behind that claim and cohort data pointing the same way. It overlaps with other tested approaches — its emphasis on viscous fibre and plant-based eating is shared with the Mediterranean pattern and the DASH diet — and its evidence is strongest for the marker it was built to move.
