Gutmaxxing: microbiome tests, probiotics and the limits of gut optimisation
The trend treats the gut as a dashboard to be tuned with home tests, probiotics and 'gut-healing' protocols. The one intervention with the broadest evidence is the least sold: fibre.
Gutmaxxing is the application of optimisation culture to digestion: treating the gut microbiome as a dashboard to be read with an at-home test and then tuned with probiotics, prebiotics, fermented foods, fibre powders and "gut-healing" protocols aimed at sealing a supposedly leaky intestinal wall. The instinct behind it is sound — the microbiome genuinely matters to health — but the tools the trend reaches for are a mismatch for what the science can currently deliver, and the one intervention with the broadest evidence is the least profitable to sell.
The test at the centre doesn't work well enough to tune anything
The gateway to gutmaxxing is usually a direct-to-consumer stool test that returns a colourful report of "good" and "bad" bacteria and personalised advice. The problem is analytical: when researchers evaluated these services by sending replicate samples, the results were not reproducible. Splitting one sample and sending it to the same company multiple times returned markedly different bacterial profiles — one firm reported 66, 72 and 121 species across three replicates of the same material — with variability on the scale of the differences between entirely different people [s1]. The clinical field agrees the tests are not ready for this use. An international consensus statement on microbiome testing concluded that there is currently no validated "healthy" microbiome to measure against and no evidence base for the personalised dietary advice these reports generate, and advised against using them to guide clinical decisions [s4]. A dashboard that gives a different reading each time it is checked cannot be optimised against, however precise its output looks.
The interventions, ranked honestly
Below the test sit the products, and their evidence varies more than the trend admits. Probiotics are the flagship, and they have genuine, specific uses — but they are narrow, strain-specific and condition-specific rather than a general upgrade, as our probiotics evidence review sets out. Fermented foods are promoted as a natural route to a "better" microbiome; the ISAPP consensus statement is supportive of their long history and biological plausibility but explicit that rigorous clinical trial evidence for specific health benefits remains limited [s2]. The item the trend underweights is the one with the strongest and least glamorous evidence: dietary fibre. It is cheap, unbranded and supported by a large body of outcome data rather than biomarker readouts, which our how much fibre piece lays out. That inversion — an expensive test and a probiotic capsule foregrounded, fibre treated as an afterthought — is the signature error of gutmaxxing.
There is also a framing problem the products rarely acknowledge. Gutmaxxing sells discrete interventions — a two-week probiotic course, a "reset" cleanse, a fermented-food challenge — but the evidence that clusters into whole-diet patterns rather than short protocols. The ISAPP consensus locates fermented foods within an overall dietary pattern rather than treating them as a standalone cure, and it is a sustained, varied, fibre-rich diet, not a one-off product cycle, that the evidence actually points toward [s2]. A protocol with a start and end date is a poor fit for a system shaped by what is eaten continuously.
"Leaky gut" and the healing protocols
Much of gutmaxxing is organised around repairing a "leaky gut", and here the trend outruns the science in a specific way. Increased intestinal permeability is a real, measurable phenomenon that appears in several diseases — but a review of the mechanisms and clinical implications concludes that whether it is a cause of illness or a consequence of it is largely unresolved in humans, and that it is not established as a standalone diagnosis driving everyday symptoms [s3]. The elaborate "gut-healing" supplement protocols sold to fix it are treating a condition whose role in health is still being worked out, with products that have not been shown to alter it.
The same caution applies to the gut-brain claims that gutmaxxing leans on — the idea that tuning the microbiome will lift mood and sharpen thinking. What to watch
Gutmaxxing gets the premise right and the execution wrong. The microbiome matters, but it cannot yet be measured reliably at home, and the products marketed to optimise it range from narrowly useful to unproven. The interventions with the best evidence — eating more fibre and a varied diet — are the ones no test is needed to justify. Persistent digestive symptoms are a reason to see a clinician who can look for a specific, treatable cause, not a prompt to buy another round of capsules.

Probiotics moved older adults' cognitive scores. Half of trials never reported harms
A 21-trial meta-analysis found effects concentrated in older age groups and largely absent elsewhere. A separate safety review of the most-used strain found no excess adverse events — and a big reporting gap.
Gymmaxxing: where the trend tracks real strength science, and where it doesn't
The training and creatine at its core rest on strong evidence. The supplement stack around them mostly doesn't — and the SARMs some users add are frequently not even the drug on the label.
The fibre that worked in IBS was psyllium, not the bran that was studied most
A meta-analysis of 30 randomised trials found fibre supplements work in IBS, with the benefit concentrated in psyllium. Wheat bran was the most-studied type and did not carry the effect.
The fibre evidence points at 25 to 29 grams a day. A supplement is not the same thing.
A Lancet series covering 135 million person-years found the largest risk reductions in that range. Trials of isolated fibre supplements produce much smaller effects on much narrower outcomes.
