Does wearing an Apple Watch improve your health? The trials say: a little, mostly steps.

Pooled across dozens of trials, activity trackers add roughly 1,800 steps a day and about 1 kg of weight loss. Effects on blood pressure, cholesterol and blood sugar are small and often not significant.

The straight answer, from the trial evidence, is: yes, a little, and mostly in the form of steps. Wearable activity trackers — the Apple Watch among them — reliably nudge people to move more in the short term. The further you go from "steps taken" toward hard health outcomes like blood pressure and blood sugar, the smaller and less certain the effects become. That is not the story the closing rings tell, but it is the one the data support.

The best available synthesis

The most comprehensive appraisal is an umbrella review — a systematic review of systematic reviews — pooling 39 reviews and meta-analyses that together drew on 163,992 participants across all age groups and both healthy and clinical populations [s1]. Its summary is worth quoting in shape: activity trackers improved physical activity, body composition and fitness, equating to approximately 1,800 extra steps per day, about 40 minutes more walking per day, and reductions of roughly 1 kg in bodyweight [s1].

Those are meaningful nudges, and cheaply obtained. But the review is equally clear about where the effect stops. For other physiological outcomes — blood pressure, cholesterol and glycosylated haemoglobin — and for psychosocial outcomes such as quality of life and pain, the effects were typically small and often non-significant [s1]. The device changes behaviour more than it changes physiology.

The same shape in older adults

A more recent meta-analysis focused on the group most often urged to use a tracker for health reasons. It combined 23 randomised trials with 4,566 community-dwelling older adults [s2]. Against usual care, it found low-to-moderate-certainty evidence that tracker-based interventions increased physical activity — and no significant effect on body composition or physical function [s2]. The pattern repeats: activity goes up, the downstream markers largely do not budge, and the confidence in even the activity effect is graded modest.

Why the effect is real but shallow

A step counter works through motivation — goals, feedback, streaks — and motivation is both the mechanism and the limitation. The nudge is real, which is why the step and activity numbers move. It also tends to fade: many trials are short, and the behaviour-change literature has long documented that novelty effects decay, so a boost measured at three months may not persist at a year. The reviews above pool trials of varying length and control conditions, which is part of why the certainty is capped at low-to-moderate.

Two caveats keep this honest. First, these syntheses evaluate activity trackers as a category; they are not trials of the Apple Watch specifically, and there is no large randomised trial showing that the Apple Watch in particular improves a hard clinical outcome. The evidence applies to the class of device, of which it is one. Second, the accuracy problems documented elsewhere on this site — that the watch's calorie and other estimates can be well off — do not undo the behavioural effect, because the nudge works even when the number is imperfect. A slightly wrong step count still gets someone off the sofa.

What to expect

Set against its price and risks, a modest, durable increase in walking is a good return, and for a sedentary person that alone is worth having — the health value of accumulating more daily steps is one of the better-established findings in the whole field. The mistake is to expect the watch to manage a condition. It will not reliably lower blood pressure, cholesterol or blood sugar on its own, and the wearable blood-pressure evidence is thinner still.

The realistic framing is a tool that helps some people move more, some of the time, with benefits concentrated in activity rather than in the biomarkers people hope to fix — and one whose motivational pull can curdle into anxiety or conflict when the numbers become the point. Worn as a prompt, it earns its place. Trusted as a treatment, it overpromises.
