Step-tracking apps add about 1,000 steps a day — for a while

Two 2026 meta-analyses put a number on it: roughly a thousand extra steps daily in older adults and in heart patients. The catch is duration — the gains faded once the intervention stopped.

Activity-tracking apps and wearables genuinely get people moving more — on the order of a thousand extra steps a day in the short term — but the effect is not shown to last once the intervention stops [s1][s2]. Two meta-analyses published in 2026, in different populations, land on strikingly similar numbers, which is the closest thing to a reliable answer this field has produced. The honest version of the claim includes the expiry date.

The numbers, from two independent syntheses

The first pooled 29 randomised controlled trials, with 3,005 participants aged 60 and older, testing activity trackers or smartphone apps to increase daily steps [s1]. In the short term — measured at the point closest to the end of the intervention — the tools increased physical activity by about 1,113 steps per day compared with a minimal intervention (95% CI 669 to 1,557, from 20 trials), and by about 912 steps per day compared with other active interventions (95% CI 412 to 1,413, from 8 trials) [s1]. The authors rated the certainty of that evidence as low and very low respectively, using the GRADE framework [s1].

The second synthesis looked at a clinical population: 14 randomised trials in patients with cardiovascular disease [s2]. Interventions built on smartphones and wearables produced a mean difference of 1,097.4 steps per day (95% CI 409.2 to 1,785.6; p=0.0018) and 3.9 additional minutes of moderate-to-vigorous physical activity per day (95% CI 0.2 to 7.6; p=0.0413) versus controls [s2]. That two separate meta-analyses — one in healthy older adults, one in heart patients — arrive at roughly the same figure of about a thousand extra steps a day is the strongest signal here [s1][s2].

A thousand steps is not trivial. It is roughly eight to ten minutes of walking, and in sedentary people it is a meaningful nudge in a direction associated with better health. But the extra 3.9 minutes of moderate-to-vigorous activity per day in the cardiac trials is a reminder of the ceiling: these tools move volume more than intensity [s2].

The catch is time

The recurring weakness is durability. The older-adults review found that only single trials reported intermediate and long-term effects — at 6, 12 and 24 months — and those suggested no sustained benefit [s1]. The authors' conclusion is explicit: tracker- and app-based interventions "may cause short-term increases in physical activity among older adults, but effects were not sustained" [s1]. This matches what longer datasets outside the trial world show. A two-year study of a commercial app that paid users small daily incentives to hit step goals found the average net gain over 24 months was small, with the biggest improvements concentrated in the least active users.

The pattern is that the device supplies novelty, structure and feedback, and behaviour responds while those are salient; when the study ends, or the habit stops feeling new, the numbers drift back. This is not a flaw unique to apps — it is the general problem of behaviour-change maintenance — but it should temper any claim that a tracker "makes" someone active in a lasting way.

What it means for a reader

An activity app is a reasonable, low-cost tool to increase walking, and the effect in the first weeks to months is real and reproducible across populations, at roughly a thousand steps a day [s1][s2]. It is not a device that installs a permanent habit; the evidence for sustained change beyond the intervention is absent, not merely weak [s1]. The same limits apply to the fitness features of consumer wearables, where the harder question — whether the tracking improves health outcomes rather than just activity — remains largely unanswered. The useful expectation is a helpful short-term push, with the maintenance left to the user.
