How much does fertility really fall with age? Less sharply than the scary numbers

Fertility does decline with age, but the most-repeated warnings overstate how steeply and how early. Outright sterility, the complete inability to conceive, stayed at about 1% and did not rise with age across the range studied [s1].

The distance between those numbers and the alarming ones is largely a distance between two kinds of study: watching real couples try, versus running a computer model. Both have their place, but they answer different questions, and the frightening version usually comes from the model.

What watching real couples showed

A prospective fecundability study recruited 782 couples from seven European natural-family-planning centres, with women aged 18 to 40 [s1]. Because the couples kept daily records of intercourse, the researchers could adjust for the timing and frequency of sex — the confounder that makes age look worse than it is when older couples simply have sex less often [s1].

The estimated share of women who failed to conceive within a year (the standard definition of infertility) was 8% at ages 19-26, 13-14% at ages 27-34, and 18% at ages 35-39 [s1]. Sterility was about 1% and unchanged with age [s1]. Male age mattered too, and later than often assumed: from ages 35 to 40, the share of couples failing to conceive within 12 cycles rose from an estimated 18% to 28%, with the man's age an important factor starting in the late 30s [s1].

Crucially, "infertile" in these figures does not mean "will never conceive." The study estimated that 43-63% of the couples who had not conceived in a year would do so if they simply kept trying for another 12 cycles [s1]. Its own conclusion was that increased infertility in older couples is "attributable primarily to declines in fertility rates rather than to absolute sterility," and that many will conceive with more time [s1].

Where the scarier numbers come from

The starker projections tend to trace to models rather than to observed couples. A widely cited modelling study used a Monte Carlo simulation combining monthly conception probabilities, miscarriage risk and age-dependent sterility to estimate outcomes [s2]. Under natural conditions it estimated that 75% of women starting to try at age 30 would have a conception ending in a live birth within one year, falling to 66% at age 35 and 44% at age 40 [s2].

Read from the other side, those figures say about a third of 35-year-olds and more than half of 40-year-olds would not have a baby within a year — numbers close to the popular warnings [s2]. Extend the window to four years and the same model gives success rates of 91%, 84% and 64% at ages 30, 35 and 40 [s2], which reframes "declining fertility" as mostly a matter of how long it takes rather than whether it happens.

The two 2004 studies are not really in conflict; they measure different things. One reports what happened to real couples within a year [s1]; the other projects lifetime and multi-year outcomes from modelled probabilities that are calibrated in part to historical natural-fertility populations [s2]. The popular "one in three at 35" statistic maps onto the model's within-a-year figure, not onto what the prospective cohort of contemporary couples observed [s1] [s2].

What the model was actually built to answer

The modelling study's real question was not "how fast does fertility fall" but "can IVF make up the difference" — and its answer was sobering in a different way. Assisted reproduction, at the success rates it assumed, made up only about half of the births lost by postponing a first attempt from age 30 to 35, and less than 30% of those lost by postponing from 35 to 40 [s2]. Its conclusion was that IVF "cannot make up for all births lost by the natural decline of fertility after age 35" [s2] — a caution against treating egg freezing or IVF as a guaranteed backstop, even as it warns against overstating the year-to-year cliff.

Both papers are now two decades old, drawn from specific European populations, and neither settles any individual's odds, which depend on health, the partner's age and much else. But together they support a more accurate summary than the headline statistics: fertility declines gradually through the 30s, most of the decline shows up as taking longer rather than never conceiving, and the most frightening single numbers describe models and historical populations more than they describe contemporary couples trying to get pregnant [s1] [s2].
