Does turmeric ease joint pain? The evidence, and a real liver risk
Small trials suggest curcumin can modestly reduce arthritis pain, sometimes on par with painkillers — but the evidence is thin, and turmeric supplements are linked to a rising number of serious liver injuries.
For osteoarthritis joint pain, turmeric's active compound curcumin has modest supporting evidence: a meta-analysis of eight small randomised trials found it reduced pain and stiffness, in some studies to a degree comparable with standard painkillers [s1]. But the trials were small and short, curcumin is poorly absorbed, and turmeric supplements have been tied to a rising cluster of serious — occasionally fatal — liver injuries, so "natural" here does not mean risk-free [s2].
What the joint-pain trials show
The best summary of the efficacy question is a 2016 systematic review and meta-analysis in the Journal of Medicinal Food, which searched a wide set of databases and found 29 candidate articles, of which eight randomised controlled trials met its criteria [s1]. Pooling the studies that used a pain visual-analogue score, three trials showed turmeric or curcumin reduced pain compared with placebo by a mean difference of 2.04 points (95% confidence interval 1.24 to 2.85) [s1]. Four trials measuring the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC), a combined score of pain, stiffness and function, found a reduction of 15.36 points with treatment (95% CI 3.77 to 26.9) [s1].
Notably, across five trials that compared turmeric or curcumin directly against pain medication, there was no significant difference in pain scores between the two [s1] — the finding behind the claim that it can work "as well as" a painkiller for some people. The doses studied corresponded to roughly 1,000 mg of curcumin a day [s1].
Why the result is weaker than it sounds
The reviewers were explicit about the limits of their own conclusion. The eight trials carried a low-to-moderate risk of bias, and the authors wrote that the number of trials, the total sample size and the methodological quality were not sufficient to draw definitive conclusions, calling for larger and more rigorous studies to confirm any therapeutic effect [s1]. Small trials of a symptom as variable and placebo-responsive as joint pain are exactly the setting where early positive findings tend to shrink on replication.
There is also a pharmacological catch that shadows every curcumin product: on its own, curcumin is poorly absorbed from the gut, which is why many supplements add black pepper extract (piperine) to boost uptake [s2]. That workaround matters for the safety story below.
The liver-injury signal
The more consequential development is not about whether turmeric helps a sore knee, but about a harm that has grown as its use has. A 2022 case series in The American Journal of Medicine, drawn from the US Drug-Induced Liver Injury Network (DILIN), identified ten cases of liver injury in which turmeric was the implicated product [s2]. All ten were enrolled since 2011, and six of them since 2017 — a clustering the authors read as a rising problem [s2].
The injuries were not trivial. Eight of the ten patients were women, the median age was 56, and the liver damage was hepatocellular in nine of the ten [s2]. Five patients were hospitalised, and one died of acute liver failure [s2]. Of the seven products chemically tested, all contained turmeric, and three also contained piperine — the same black-pepper additive used to raise absorption, which the authors suggest may increase exposure to the compound [s2]. The typical latency was one to four months from starting the supplement to injury [s2].
A striking genetic thread ran through the cases: seven of the patients carried the immune-system gene variant HLA-B*35:01, giving an allele frequency of 0.450 against a population background of 0.056 to 0.069 [s2]. That points to an idiosyncratic, immune-mediated reaction in susceptible people rather than a simple dose-toxicity effect — which is precisely the kind of harm a consumer cannot predict in advance.
How to weigh it
Two things are true at once. Curcumin has genuine, if modest and shakily evidenced, support for easing osteoarthritis symptoms in short trials [s1]. And turmeric supplements — concentrated extracts, often enhanced for absorption — carry a real, if uncommon, risk of serious liver injury that appears to be increasing [s2]. Culinary turmeric in food is not what these injury cases describe; the concern is with high-dose supplements.
