Does stevia mess with your blood sugar or hormones? A crossover trial says no

In 69 people across normal weight, overweight and type 2 diabetes, a stevia drink behaved like water — no shift in glucose, insulin, gut hormones, appetite or how much they ate next.

A recurring worry about non-nutritive sweeteners is that, even though they carry no sugar, they might quietly disturb metabolism — spiking insulin, nudging the hormones that govern appetite, or leading people to eat more later. A crossover trial published in The Journal of Nutrition on 1 September tested that worry for stevia, a plant-based sweetener, and found little to support it [s1].

The claim being tested

The idea that a sweet taste without calories could still trigger a hormonal or appetite response is plausible on its face, and it is the mechanism behind a lot of sweetener anxiety. The authors note that the acute effects of stevia on blood glucose and hormonal responses have not been fully investigated — which is the gap this study set out to fill [s1].

What they did

The trial enrolled three cohorts of 23 people each — with normal weight, with overweight, and with type 2 diabetes — for a total of 69 participants [s1]. In a randomised-sequence crossover design, each person completed four test conditions: a drink with stevia (75.6 mg of steviol equivalents), water, glucose (30 g), and stevia combined with glucose (30 g glucose plus 75.6 mg steviol equivalents) [s1].

Blood was sampled before and for 180 minutes after each drink [s1]. The researchers tracked plasma glucose, insulin, and the gut and pancreatic hormones that regulate blood sugar and appetite — glucose-dependent insulinotropic polypeptide (GIP), glucagon-like peptide-1, glucagon and PYY — using the area under the response curve and peak concentrations [s1]. They also measured appetite with visual-analogue scales and, crucially, how much participants ate at a subsequent free-access meal [s1].

The result: stevia looked like water

The response to the stevia drink did not differ significantly from water alone, for any of the three groups [s1]. And when stevia was added to glucose, the combination did not differ significantly from glucose alone [s1] — meaning stevia neither produced a response of its own nor amplified or blunted the body's response to actual sugar.

Appetite ratings and the amount eaten at the later meal were likewise unchanged [s1]. In the authors' summary, acute stevia consumption did not materially alter glycaemia, the hormones involved in glucose and appetite regulation, appetite ratings, or energy intake at a subsequent meal [s1].

What it does and doesn't settle

This is a well-designed acute trial: crossover, randomised in sequence, with each participant serving as their own control across four conditions, and importantly it included people with type 2 diabetes rather than only healthy volunteers [s1]. For the specific fear it addresses — that a single dose of stevia sets off an insulin or appetite reaction — the answer here is reassuringly flat.

The word doing the work is acute. This trial measured the three hours after one drink; it is not a study of what happens when stevia replaces sugar in the diet over months, nor of long-term weight, cardiometabolic or gut-microbiome effects, which are where the larger and less settled debates about sweeteners actually sit. The dose was a defined amount of steviol glycosides in a beverage, not the full range of ways stevia reaches people in processed food. And a null result in 69 people, while informative, is not proof of no effect — only of no effect large enough to show up in this sample.

So the popular claim — that stevia secretly spikes your hormones or blood sugar in the moment — does not hold up in this test. The broader question of how non-nutritive sweeteners affect health when used habitually is a separate one, and this study does not answer it. As with any dietary choice, that is a question to work through with a clinician rather than a single trial.

The trial was prospectively registered on ClinicalTrials.gov (NCT05287906) [s2], and its crossover structure — each participant completing all four drink conditions — is a genuine strength, since it removes the between-person differences that complicate parallel-group comparisons of diet. Even so, a reader should keep the scope in view: this concerns the immediate physiological response to a single, defined dose, and it answers a narrow fear rather than the wider, unsettled question of what habitual sweetener use does over years [s1].
