Loneliness is not literally 'as deadly as 15 cigarettes.' The evidence is still striking.

The famous cigarette comparison is a communications gloss on a robust but observational finding: people with stronger social ties are markedly more likely to survive follow-up.

You have probably heard that loneliness is "as bad for you as smoking 15 cigarettes a day." It is one of the most repeated statistics in public health, and it is more a vivid translation than a measured fact. The underlying evidence is real and genuinely important — social connection is robustly associated with living longer — but the cigarette equivalence is a communications device layered on top of it, and worth understanding for what it is.

Where the number comes from

The "15 cigarettes" line was propelled into wide circulation by the 2023 U.S. Surgeon General's advisory on loneliness and isolation, which stated that the mortality impact of being socially disconnected is similar to that of smoking up to 15 cigarettes a day, and attributed the comparison to the work of researcher Julianne Holt-Lunstad [s3]. That advisory is a synthesis of the science, not the original measurement — and tracing the claim back to its source shows what kind of number it actually is.

What the source studies measured

The foundational study is a 2010 meta-analysis pooling 148 studies and 308,849 participants [s1]. It found that people with stronger social relationships had a 50% greater likelihood of survival over the follow-up periods studied — an odds ratio of 1.50 (95% confidence interval 1.42 to 1.59) [s1]. The effect was largest for complex measures of social integration (odds ratio 1.91) and smallest for the crude binary of living alone versus with others (odds ratio 1.19, with a confidence interval that crossed into non-significance) [s1].

Crucially, the authors' own framing of the magnitude was that the influence of social relationships on mortality risk is "comparable with well-established risk factors for mortality" [s1]. That comparison — social connection sits in the same league as the big, familiar killers, smoking among them — is the seed from which the "15 cigarettes" equivalence grew. It is a statement about comparable effect sizes across risk factors, not a measured finding that a specific amount of loneliness equals a specific number of cigarettes.

A second meta-analysis, in 2015, looked specifically at isolation and loneliness rather than at connection, and found more moderate figures: social isolation was associated with a 29% increased likelihood of mortality (odds ratio 1.29), loneliness with 26% (1.26), and living alone with 32% (1.32) [s2]. Notably, it found no meaningful difference between objective isolation and the subjective feeling of loneliness in predicting death [s2].

Why the cigarette framing is a gloss, not a measurement

Two things separate the headline from the evidence. First, "15 cigarettes a day" is an equivalence in effect size translated into an intuitive unit; nobody ran a study assigning people to loneliness or to smoking and comparing death rates directly. It converts an odds ratio into a smoking-dose image because that image communicates, not because the data are expressed that way. Second, the size of the loneliness effect depends heavily on how it is measured — the 2010 and 2015 analyses produced different numbers for related but distinct constructs [s1] [s2] — so a single fixed cigarette count papers over a range.

Then there is the perennial caveat: these are observational studies. Sick and dying people become more isolated, so some of the association runs from ill health to loneliness rather than the reverse. The meta-analyses adjust for confounders and note that results hold across age, sex and follow-up length, which strengthens the case, but no meta-analysis of observational data can fully establish that loneliness causes earlier death.

What survives the scrutiny

Stripping away the cigarette metaphor leaves a finding that is still one of the more robust in social epidemiology: across hundreds of studies and hundreds of thousands of people, stronger social relationships are consistently associated with a substantially higher chance of survival, on a scale comparable to major established risk factors [s1] [s2]. That is a serious result, and it is the reason a Surgeon General issued an advisory about it at all [s3].

The honest correction is narrow. The relationship between social connection and longevity is real, large and worth taking seriously. The specific, catchy "15 cigarettes a day" is a simplification of a comparison between effect sizes — useful for conveying scale, misleading if taken as a precise, measured exchange rate between loneliness and tobacco.
