What 'fragrance' hides: household VOCs, the evidence, and where it thins out

A single word on a label can stand for dozens of unlisted compounds. A few, like formaldehyde, are established hazards; the claim that everyday scented products make the general public ill rests mostly on self-report.

The word "fragrance" on a household product label can stand for dozens of individual chemicals that manufacturers are not required to name, and the honest state of the evidence is mixed: some of the compounds emitted are established hazards at sufficient exposure, while the broader claim that everyday scented products sicken the general population rests largely on self-reported symptoms that are hard to attribute [s1][s3][s4]. Cleaners, air fresheners, laundry products, paints and new furnishings all release volatile organic compounds — VOCs — indoors, where concentrations typically run higher than outdoors [s2].

What the label leaves out

The clearest documented problem is disclosure. An emissions study of fragranced products found that only about 5% of the 684 VOCs detected — and only 12% of the 207 classified as potentially hazardous — were listed anywhere on the label, safety data sheet or website, and that more than 95% of both conventional and "green" products emitted at least one potentially hazardous VOC [s1]. That does not mean those emissions cause disease at the levels a consumer meets; it means a shopper cannot tell from the packaging what a product actually releases [s1].

The compounds that are established hazards

Some individual VOCs found indoors have solid evidence behind them. Formaldehyde, emitted by some pressed-wood furniture, and present in some cleaning and personal-care products, is classified by the International Agency for Research on Cancer as carcinogenic to humans, with sufficient evidence for nasopharyngeal cancer and leukaemia — a Group 1 classification, the same category as tobacco smoke and asbestos [s3]. That classification reflects the hazard of the chemical, established largely in occupational settings; it is a statement about what formaldehyde can do, not a measure of the risk from a given air freshener [s3]. The EPA notes that VOC exposure can cause eye, nose and throat irritation, headaches and, for some compounds at higher exposures, more serious effects [s2].

Where the evidence thins

The gap is between those specific, well-characterised hazards and the popular framing that scented products broadly harm health. Population surveys report that large minorities say they experience adverse effects from fragranced products: one nationally representative Australian survey found 6.5% reported medically diagnosed multiple chemical sensitivity and 18.9% reported being unusually sensitive to everyday chemicals, with 91.5% of the MCS group reporting fragrance-triggered symptoms such as migraine [s4]. But these are self-reported associations, not controlled measurements of exposure and outcome, and multiple chemical sensitivity itself remains a contested diagnosis without an agreed biological mechanism [s4]. Symptoms are real to those experiencing them; what the data cannot do is prove that a specific product at household concentrations caused a specific illness in the general population [s4].

What the evidence supports doing

The interventions that follow from the solid parts of the evidence are unglamorous: ventilate when using cleaning products, paints or new furnishings; favour products that disclose their ingredients; and, for people who report sensitivity or have asthma, reduce use of strongly scented products, for which the rationale is symptom avoidance rather than proven prevention of disease [s1][s2]. Reducing indoor VOC concentrations is a defensible general goal because the sources are controllable and the highest-exposure compounds carry real hazard [s2][s3].

The reporting takeaway is to hold two things at once: the disclosure problem is documented and the worst-characterised compounds are genuinely hazardous, but the sweeping health claims made for and against "fragrance" outrun what the population-level evidence can support [s1][s3][s4].
