Carbon monoxide kills about 1,000 Americans a year. An alarm is the warning
The gas is colourless, odourless and produced by any fuel-burning appliance. Because the early symptoms mimic flu, a working CO alarm is the one defence public-health data support.
Carbon monoxide is a colourless, odourless gas produced whenever fuel burns incompletely — from furnaces, water heaters, gas ranges, fireplaces, generators and car engines — and a working CO alarm is the only reliable way to detect it before it causes harm [s3]. Because the early symptoms of poisoning are non-specific — headache, dizziness, nausea, confusion — they are routinely mistaken for flu or fatigue, which is exactly why the gas is dangerous inside homes [s3].
The US mortality burden is real but often misstated. A national analysis of death-certificate data found total carbon monoxide deaths fell from 1,253 in 2015 to 1,067 in 2021, but that accidental CO deaths rose over the same period, and in 2021 accidental deaths (543) outnumbered intentional ones (524) for the first time on record [s2]. A separate surveillance study combining poison-centre calls, emergency-department visits, hospitalisations and deaths for 2005–2018 estimated an annual unintentional CO death rate of about 3.3 per million people, with non-fire-related cases making up 40.9% of those deaths [s1].
Who is most exposed
The same surveillance study found the pattern of harm varies by age. Children aged 0 to 9 had the highest reported rates of poison-centre exposure calls and emergency-department visits, at 54.1 and 70.5 per million respectively, while adults older than 80 had the highest rates of hospitalisation and death [s1]. That split — young children over-represented in exposures, older adults over-represented in deaths — reflects both how CO is encountered and how poorly older bodies tolerate it [s1].
What the evidence supports doing
The intervention with the clearest rationale is detection. CO alarms are inexpensive and are the only device that reveals a leak while it can still be acted on, since the gas gives no sensory warning [s3]. The Consumer Product Safety Commission advises installing alarms on every level of a home and near sleeping areas, so a rising concentration wakes occupants overnight — the period when fatal poisonings most often occur because sleepers do not notice symptoms [s3]. Alarms have a limited service life and are replaced on the schedule printed on the unit [s3].
The second line of defence is prevention at the source: having fuel-burning appliances and chimneys inspected, never running a generator, grill or car engine in an enclosed or attached space, and not using a gas oven to heat a room [s3]. Portable generators are a recurring cause of clusters of poisonings during power outages, which is why guidance stresses keeping them well away from windows, doors and vents [s3].
The limits of the numbers
Surveillance data undercount CO poisoning because mild cases resolve without a diagnosis and the symptoms are easily attributed to other illnesses [s1]. Death-certificate totals also combine unintentional exposures with suicides, which behave differently and respond to different interventions — a distinction the 2021 crossover between accidental and intentional deaths makes concrete [s2]. What the evidence does not do is quantify precisely how many deaths a CO alarm prevents in a given home; the case for alarms rests on the gas being undetectable by human senses and the poisoning being preventable once detected, rather than on a randomised trial [s1][s3].
