Halfway through 2026, the United States has all but matched an entire prior year's measles caseload. The numbers behind the milestone
The case count spans 41 jurisdictions, and 93 percent of this year's cases are linked to active, identified outbreaks rather than isolated importations [s1]. Vaccination status tracks with that pattern: 93 percent of 2026 cases have occurred in people who were unvaccinated or whose vaccination status is unknown, while only 4 percent occurred in people who had received both recommended doses of MMR vaccine [s1].
The age distribution skews young. Children and teenagers aged 5 to 19 account for 51 percent of cases, and children under 5 make up another 20 percent [s1]. That youngest group also carries the sharpest severity signal in the data: 10 percent of infected children under 5 have required hospitalization, against an overall hospitalization rate of 6 percent — 138 people — across all 2026 cases [s1]. No deaths have been reported so far in 2026, compared with three in all of 2025 [s1].
Why this was foreseeable, not sudden
The rise is not a new development so much as the continuation of a trend that has been building for several years. Measles elimination in the US — the status the country has held since 2000, defined as the absence of endemic transmission for at least 12 consecutive months — depends on vaccination coverage staying high enough, and consistently enough across communities, to prevent chains of transmission from sustaining themselves once a case is imported. National kindergarten MMR coverage has been sliding: from 95.2 percent in the 2019-2020 school year to 92.5 percent in 2024-2025 [s1]. The 95 percent threshold is the commonly cited benchmark for herd immunity against measles, one of the most contagious pathogens known; each percentage point below it widens the population of people who can sustain an outbreak once one starts.
That decline is national and gradual, but its effects concentrate unevenly. A national average above 92 percent still leaves room for pockets of much lower coverage — specific counties, school districts or communities — where an imported case can find enough susceptible people to take hold and spread for months, which is the dynamic behind the 93 percent of cases now linked to identified outbreaks rather than isolated travel-related infections.
What the milestone does and doesn't mean
Crossing the prior year's full-year total in early July is a data point, not a verdict. It says the current outbreak dynamics are worse, cumulatively, than last year's — a year that was itself the highest since the elimination declaration. It does not by itself determine whether the US will lose its formal elimination status, which hinges on a separate, technical assessment of whether any single chain of transmission has continued unbroken for 12 months. That determination is a distinct process running on its own timeline.
What the numbers do make clear is the mechanism driving the increase: this is overwhelmingly a story about unvaccinated and under-vaccinated populations sustaining outbreaks once measles arrives, not about breakthrough infections in vaccinated people. The 4 percent of cases occurring in fully vaccinated individuals is consistent with the vaccine's known, high — but not absolute — effectiveness, and does not indicate any change in how well MMR protects those who receive it.
What to watch
Whether the case count's growth rate holds steady, accelerates, or slows through the remainder of the year, and whether new outbreaks emerge in additional jurisdictions beyond the 41 already reporting cases. Hospitalization trends among children under 5 are also worth tracking closely, since that group is both the most severely affected and the group for which vaccination timing is most consequential.
