How worried should people be about H5N1 bird flu? What the risk assessment says

The honest position is two things at once: the current risk to the general public is low, and the reason it is watched so closely is that its worst-case potential is high. The gap between the two is the whole story.

The question people ask about H5N1 bird flu — how worried should I be? — does not have a single number for an answer, and the reason is worth understanding, because it recurs with every emerging pathogen. The risk of a disease is roughly its probability multiplied by its severity, and for H5N1 those two terms point in opposite directions. The probability that any given member of the public catches it right now is low; the severity if the virus were to change in the wrong way is potentially very high. Holding both facts at once is not fence-sitting. It is the actual state of the evidence.

Why the current risk to the public is low

H5N1 is, first and foremost, a disease of birds. The World Health Organization's assessment is that human infections occur mainly through direct or close contact with infected animals or contaminated environments, and that — critically — the virus does not currently transmit easily from person to person [s1]. That last point is the load-bearing one. Almost every human case on record has been traced to animal exposure, not to catching it from another person, and there has been no sustained human-to-human chain [s1]. A virus that cannot readily pass between people cannot start a pandemic, however dangerous it is to the individuals who do catch it. For someone with no contact with sick poultry, dairy cattle or other infected animals, that is why public-health agencies describe the present risk to the general population as low [s1].

The 2024 spillover into dairy cattle in the United States tested this picture and, so far, has largely confirmed it. Human infections among exposed farm workers have occurred, but the first detailed case reported in the New England Journal of Medicine was mild — the worker's main sign was conjunctivitis, an eye infection, and they recovered [s2]. Several subsequent worker cases followed a similar mild, mostly eye-focused pattern. That is reassuring at the level of what has happened, and it is not the same as a guarantee about what could.

Why it is watched so closely anyway

The reason H5N1 commands so much attention despite a low current public risk is the severity term. In the human cases reported to WHO over the past two decades — a group heavily weighted toward severe, hospitalised infections detected through surveillance — the case-fatality proportion has been high [s1]. That figure has to be read with care: mild and asymptomatic infections are far more likely to go undetected than fatal ones, so the true fatality rate of any given infection is almost certainly lower than the reported proportion suggests. But even with that correction, H5N1 has demonstrably killed a substantial share of the people it has made seriously ill [s1], which is a different and graver profile than seasonal flu.

Layered on top is pandemic potential. Influenza viruses evolve, and they can swap gene segments when two strains infect the same host — a process that could, in principle, hand H5N1 the ability to spread between people that it currently lacks. That this is not merely theoretical was shown over a decade ago by controversial laboratory experiments, published in Science, demonstrating that a small number of mutations could make an H5N1 virus transmissible by air between ferrets, the standard animal model for human flu transmission [s3]. Those experiments were about what is possible under engineered conditions, not a prediction of what will happen in the wild [s3]. But they are why "it does not spread between people yet" is watched with the emphasis on yet.

What the balance actually means

Put together, the assessment is coherent even though it resists a one-word answer. The features that would make H5N1 a public emergency — easy, sustained human-to-human transmission — are not present, which is why routine anxiety is not warranted for people without animal exposure [s1]. The features that make it worth intensive surveillance — high severity in serious cases and a plausible, if uncertain, route to greater transmissibility — are present, which is why scientists monitor it far more closely than a low current case count alone would justify [s1][s3]. The thing to watch is not the raw number of animal outbreaks but any sign of efficient spread between people; that, not the current headlines, is what would change the calculation. This is an evolving situation, and the figures and assessments here reflect the evidence available at the time of writing rather than a fixed forecast.
