Can a cycle-tracking app work as birth control? The evidence, honestly

In a 22,785-user study of one temperature-based app, typical use failed 6.9 per 100 woman-years and perfect use 1.0 — but the study came from the maker, and traditional methods vary widely.

A cycle-tracking app used strictly as a fertility-awareness method can prevent pregnancy at rates comparable to other user-dependent methods — but only under strict use, and the best-published numbers come from a study the app's own makers conducted [s1]. The broader evidence on fertility-awareness methods is thin and uneven, with real-world failure rates that vary widely from one method and study to the next [s2]. That is the honest state of the question, and it matters because "period tracker" and "birth control" are not the same claim.

Most popular cycle apps are not contraceptives at all; they predict periods and ovulation windows for planning. A small number are designed and marketed as contraception, using logged data to tell a couple which days require a barrier method or abstinence. The evidence below concerns that second, narrower category.

What the app study found

The most-cited study of a contraceptive app followed 22,785 users of Natural Cycles, a method that combines the dates of menstruation with daily basal body temperature to flag fertile and infertile days [s1]. Users logged a total of 18,548 woman-years of data, which the researchers used to calculate Pearl Indexes — pregnancies per 100 woman-years of use [s1].

Typical use produced a Pearl Index of 6.9 pregnancies per 100 woman-years (95% CI 6.5-7.2), and the study estimated a 13-cycle typical-use failure rate of 8.3% [s1]. Perfect use — following the app's guidance every cycle — gave a Pearl Index of 1.0 (95% CI 0.5-1.5) [s1]. The rate of pregnancies traceable to the algorithm itself wrongly flagging a fertile day as infertile was 0.5 per 100 woman-years [s1]. The authors concluded the app's efficacy is "higher than usually reported for traditional fertility awareness-based methods" [s1].

Two numbers in the same study temper that pitch. The gap between the 1.0 perfect-use and 6.9 typical-use Pearl Index shows how much depends on the user acting on the app's warnings [s1]. And the discontinuation rate over 12 months was 54% — more than half of users stopped within a year [s1].

The conflict that has to be stated

This study cannot be read like an independent trial. Its author list includes the app's co-founders, and the corresponding author's affiliation is the company itself [s1]. That does not make the numbers wrong, but it places them in the category of manufacturer-reported evidence, where independent replication matters more than usual — the same standard this newspaper applies to a drug company's own trial of its own product.

What the independent evidence says about the wider method

Fertility-awareness methods as a class have a weaker evidence base than the app study alone suggests. A systematic review in Obstetrics & Gynecology set out to summarise the best prospective data on typical- and perfect-use effectiveness across specific methods [s2]. Of 53 studies it examined, it rated none high quality — 21 were moderate and 32 low [s2].

Among the moderate-quality studies, first-year typical-use pregnancy rates varied widely between methods, and the reviewers' overall conclusion was blunt: studies on the effectiveness of each fertility-awareness method "are few and of low to moderate quality" [s2]. For the Standard Days Method, moderate-quality studies put first-year typical-use pregnancy rates at 11.2 to 14.1 per 100 woman-years and perfect use at 4.8 [s2]. The point of the review is not any single figure but the spread: a method's published effectiveness depends heavily on which study, which population and which teaching you look at [s2].

What this leaves a reader with

A dedicated contraceptive app, used strictly, performs in the range of other user-dependent methods, and better than older fertility-awareness techniques in its maker's own data [s1]. Used loosely — days skipped, warnings ignored — it performs like the typical-use figure, and half of users abandon it within a year [s1]. The independent literature on fertility awareness as a whole remains limited and of modest quality [s2]. None of this speaks to a general period-tracking app, which makes no contraceptive claim and should not be treated as one.
