An estrogen-metabolism lifestyle score tracked with lower breast and uterine cancer risk

Women whose habits aligned with a favorable estrogen-metabolism pathway had up to 61% lower endometrial cancer risk. The score explains only a tenth of the variation it predicts.

Estrogen doesn't just circulate in the body — it's metabolized along different chemical pathways, and one of those pathways, called 2-hydroxylation, produces metabolites believed to have anti-tumor properties, in contrast to other pathways associated with cancer promotion. A study published this month in Cancer Prevention Research built a lifestyle score correlated with that favorable metabolic pathway and tested whether it predicted lower cancer risk in two large cohorts of postmenopausal women [s1].

The design

Researchers first derived lifestyle scores using elastic net regularized regression — a statistical method for identifying which of many lifestyle factors best predict a target measure — correlating lifestyle patterns with the ratio of 2-hydroxylation estrogen metabolites to parent estrogens [s1]. This derivation step used smaller subsets of cancer-free postmenopausal women: 723 from the Shanghai Women's Health Study (SWHS) and 635 from the Prostate, Lung, Colorectal and Ovarian Screening Trial (PLCO), whose urine or blood samples, respectively, were profiled for these estrogen metabolites at baseline [s1]. The resulting lifestyle scores were then applied to the full cohorts — 35,453 women in SWHS and 26,565 in PLCO — using Cox proportional hazards models to estimate the association between lifestyle score and risk of breast, endometrial, and ovarian cancer [s1].

How much the score actually explains

Before the cancer results: the lifestyle scores explained only 10–11% of the total variance in the actual estrogen-metabolite ratio they were built to predict [s1]. That's a meaningful caveat — the score is a modest proxy for the underlying biology, not a precise measurement of it, which matters when interpreting how strongly it associates with cancer outcomes downstream.

What it found

In the Shanghai cohort, women in the highest quartile of the lifestyle score had 29% lower risk of breast cancer (hazard ratio 0.71, 95% CI 0.57–0.88, p for trend < 0.001) and 61% lower risk of endometrial cancer (hazard ratio 0.39, 95% CI 0.21–0.71, p for trend < 0.001) compared with the lowest quartile [s1]. The breast cancer association was more pronounced for estrogen receptor-positive tumors than estrogen receptor-negative ones [s1]. Similar inverse associations — lower cancer risk with higher lifestyle scores — were observed in the separate PLCO cohort [s1], though the abstract does not report the PLCO-specific hazard ratios.

Reading this alongside the 10% figure

The paper's own numbers create some tension worth naming directly: a lifestyle score that captures only 10–11% of the variance in its target biological measure nonetheless shows a fairly strong statistical association with cancer risk — a 61% lower endometrial cancer risk at the top quartile is a large effect size for an epidemiological association. That combination is possible, but it also raises the question of whether the lifestyle score is capturing cancer-protective behaviors more broadly — diet quality, physical activity, body weight, alcohol use, whichever specific factors feed into the score — rather than specifically operating through the estrogen-metabolism pathway the study is built around. The study's design, built to test a specific metabolic hypothesis, doesn't fully rule out that its lifestyle score is a proxy for general healthy-lifestyle behaviors that have many other established links to cancer risk independent of estrogen metabolism.

What this doesn't establish

This is observational cohort data, not an intervention trial — nobody was randomized to follow the favorable lifestyle pattern, so causation isn't established, and confounding by other health behaviors correlated with the score remains possible. The metabolite-ratio measurements used to derive the score came from small subsets of each cohort (723 and 635 women), with results then extrapolated to the full cohorts based on lifestyle factors alone, not repeated hormone measurements in everyone.

What to watch

Whether the specific lifestyle components driving the score are identified and tested in an intervention study, and whether the association holds after adjusting more completely for other known cancer risk factors correlated with healthy lifestyle. This article is not medical advice.
