The rulebook governing athlete drug testing has been rewritten. It takes effect in 2027.
Anti-doping is defended mainly as a health protection regime. The 2027 Code approved in Busan adds an intelligence standard and rebuilds the data-protection rules — and leaves the hardest health question untouched.
The document that determines what an athlete may put in their body, who may test them for it, and what happens to the sample afterwards has been rewritten. On 5 December, at the close of the Sixth World Conference on Doping in Sport in Busan, the World Anti-Doping Code 2027 and a revised set of International Standards were approved [s1].
They enter into force on 1 January 2027 [s1]. That gap is deliberate: every signatory — international federations, national anti-doping organisations, national Olympic committees — has roughly a year to align its own rules with the new framework before it binds them [s1].
What was approved
The conference ran from 2 to 5 December in Busan, Republic of Korea, and drew representatives from more than 190 countries, including anti-doping organisations, federations, governments and athletes [s1]. It closed with the adoption of a "Busan Declaration" calling on the anti-doping community to strengthen cooperation, expand education, and improve investigations and sanctions [s1].
The final drafts had been published on 14 November, ahead of the vote [s2]. Nine documents make up the package: the Code itself, plus International Standards covering code compliance, data protection, education, intelligence and investigations, laboratories, results management, testing, and therapeutic use exemptions [s2].
Two of those are structurally new. The International Standard for Intelligence and Investigations did not previously exist as a standalone standard [s2]. And the old International Standard for the Protection of Privacy and Personal Information has been rebuilt as an International Standard for Data Protection [s2]. The testing standard has been renamed as well, from the International Standard for Testing and Investigations to the International Standard for Testing — a consequence of splitting investigations out [s2].
The revision ran two years across three consultation phases and drew feedback from more than 600 athletes in over 60 countries [s2]. WADA's stated objectives for the 2027 Code include strengthening protection for young and vulnerable athletes, improving transparency and fairness, and reinforcing international cooperation [s1].
Why a health desk covers this at all
Anti-doping is usually reported as a sports-integrity story. Its public justification is substantially a health one.
The case, as anti-doping bodies make it, is that the substances on the prohibited list have demonstrated harms, some of which do not resolve when the drug is stopped, and that the combined effects of taking several prohibited substances simultaneously — which is what high-level doping usually involves — have not been characterised at all [s3]. That second point is the more honest one. There is no body of trial evidence on stacked supraphysiological dosing in elite athletes, because such a trial could not be run.
Which cuts both ways. The absence of evidence is a reason for caution, and it is also the reason that the health argument against doping is generally an argument from pharmacology and case reports rather than from controlled data.
What the new framework does and does not change
The data-protection standard is the quietly consequential one. Anti-doping generates an unusual volume of sensitive personal information: whereabouts filings that locate an athlete at a specified hour every day, therapeutic use exemption files that disclose diagnoses, longitudinal biological passport data. Rebuilding that standard as a data-protection instrument rather than a privacy-and-personal-information instrument [s2] is a signal about how the system now understands its own exposure, though what it delivers in practice will depend on text that has to be read against each signatory's national law.
The intelligence and investigations standard reflects a longer shift in how doping cases are actually made — increasingly through non-analytical evidence rather than a positive test [s2].
What has not changed is the underlying difficulty. Anti-doping asks athletes to accept an extraordinary level of surveillance and a strict-liability standard, justified partly by health protection, in a system whose health evidence base is thin by design. The 2027 Code tightens the machinery. It does not resolve that.
What to watch
Through 2026, signatories will publish rules aligned to the new Code, and the differences between them will be where the practical consequences show up — particularly on results management timelines and on how the data-protection standard is implemented in jurisdictions with strong domestic privacy law.
The Prohibited List is a separate annual instrument and continues to be revised on its own cycle, independent of the Code revision.
