What ALT and AST measure on a blood test, and why one high value is not a diagnosis
Raised liver enzymes are common and mean the liver may be under strain, not that a disease is confirmed. The pattern, degree and duration of the change are what a clinician reads, not a single flag.
If a routine blood test comes back with a raised ALT or AST, the most important thing to know is that it flags possible strain on the liver — it is not, on its own, a diagnosis of liver disease [s1]. These are among the most commonly ordered blood tests, mildly abnormal results are common, and what a clinician actually interprets is the pattern of the numbers, how high they are, and whether they persist — not a single value in isolation [s1] [s2].
What the enzymes actually are
ALT (alanine aminotransferase) and AST (aspartate aminotransferase) are enzymes found inside cells. When liver cells are injured, these enzymes leak into the bloodstream, so a rise in them is a marker of hepatocellular injury — damage to the liver cells themselves [s1]. The American College of Gastroenterology, in its clinical guideline on evaluating abnormal liver chemistries, groups ALT and AST with two other common measures — alkaline phosphatase and bilirubin — and makes a point of terminology: these should be called liver chemistries or liver tests, not "liver function tests," because a raised enzyme reflects injury rather than how well the liver is doing its job [s1].
The distinction the guideline draws next is the one that carries most of the diagnostic weight. A disproportionate rise in AST and ALT relative to alkaline phosphatase points to hepatocellular injury — the liver cells. A disproportionate rise in alkaline phosphatase relative to AST and ALT points instead to a cholestatic pattern — a problem with bile flow [s1]. Bilirubin can rise in either. So the same set of four tests reads in two very different directions depending on which values move together, which is why a lab printout with one starred number tells a reader far less than the shape of the whole panel tells a clinician [s1].
What "normal" means, and why the range is contested
The reference range printed next to the result is not a fixed line between health and disease. The ACG guideline notes that a true healthy-normal ALT is lower than many laboratories report — in the range of 29 to 33 IU/L for men and 19 to 25 IU/L for women — and that values above this warrant assessment [s1]. Laboratory upper limits have historically been set from populations that included people with undiagnosed fatty liver, which pushes the printed "normal" ceiling upward. That is why two labs can call the same result normal or abnormal, and why the number matters less than the trend.
The degree of elevation guides the workup. The guideline states plainly that how high the ALT or AST rises helps direct the evaluation — a mild, long-standing elevation prompts a different set of questions than a very high, sudden one [s1]. A large, acute spike suggests things like acute viral hepatitis, a drug or toxin injury, or reduced blood flow to the liver; a mild chronic elevation more often traces to common conditions such as fatty liver disease or alcohol [s1].
Why a single value gets over-read
Raised liver enzymes are worth taking seriously, and the reason is buried in the evidence: the ACG guideline notes that multiple studies have linked an elevated ALT to increased liver-related mortality [s1]. But "worth investigating" is not the same as "diagnosed." The British Society of Gastroenterology, whose 2018 guideline covers the management of abnormal liver blood tests in both primary and secondary care, built its recommendations around exactly the questions a lone flagged value cannot answer: what constitutes an abnormal result, when tests should be checked at all, and whether the extent and duration of the abnormality should determine what happens next [s2]. The framing is deliberate. An isolated, mildly abnormal reading is a prompt to look further in context, not a verdict — and the guideline was written in part because such results are both common and easy to mishandle in either direction, over-investigated in some patients and ignored in others [s2].
What the evaluation looks for
When hepatocellular injury is the pattern, the ACG guideline lays out a standard sweep of causes to test for rather than a single answer: viral hepatitis A, B and C; fatty liver disease and alcohol-related liver disease; and less common but important conditions including hereditary haemochromatosis, autoimmune hepatitis, Wilson's disease and alpha-1 antitrypsin deficiency [s1]. It also specifies taking a careful history of prescription and over-the-counter medicines, because drugs and supplements are a frequent and reversible cause [s1]. A cholestatic pattern sends the workup in a different direction, toward conditions such as primary biliary cholangitis and primary sclerosing cholangitis, and an isolated raised bilirubin is fractionated to work out whether it reflects liver disease at all [s1]. Only when blood tests and imaging fail to settle the question does the guideline raise a liver biopsy [s1].
What this leaves a reader with
A raised ALT or AST says the liver may be under strain and the cause is worth pinning down. It does not say which cause, how serious it is, or whether it will persist — those are read from the pattern across the panel, the size of the rise, whether it repeats on a second test, and the person's history [s1] [s2]. A result flagged on a lab report is a reason to have the specific numbers and their context discussed with a clinician, who can decide whether it is a transient, benign finding or the start of a workup. It is not a number a reader can safely translate into a diagnosis alone.
