FDA wants a 'Low/Med/High' box on food fronts. Does that kind of label work?
The proposed Nutrition Info box would rate saturated fat, sodium and added sugars on the front of most packaged foods. The strongest real-world evidence is for blunter warning labels.

The FDA has proposed requiring a standardized "Nutrition Info" box on the front of most packaged foods, rating their saturated fat, sodium and added sugars as "Low," "Med" or "High" [s1]. The evidence that front-of-package labels change what people buy is real, but it turns heavily on design — and the largest real-world effects come from the blunt warning labels the FDA chose not to adopt [s1][s3].
What was proposed
Published in the Federal Register on 16 January 2025, the proposed rule would require a compact box on the principal display panel — the front — of most foods that already carry a Nutrition Facts label [s1]. For each of the three "nutrients to limit," the box would show the amount per serving, the percent Daily Value, and an interpretive word: Low, Med or High [s1]. FDA proposes staggered compliance: three years for businesses with $10 million or more in annual food sales, and four years for smaller ones [s1]. The comment period, originally set to close in May, was extended to 15 July 2025 [s2].
The agency's stated problem is that the current label leaves too much interpretation to the shopper. FDA cites data that up to 40 percent of American consumers misinterpret or do not understand the percent Daily Value figure that already appears on the back of the pack [s1]. A front-of-pack cue, the theory goes, does the comparison for them at the moment of choice.
Why "Low/Med/High" and not a warning
FDA says it tested several formats and that the proposed Nutrition Info box "performed best among all schemes tested in helping consumers identify healthier nutrient profiles" — the agency's own finding from its consumer research [s1]. That is a meaningful claim, but it measures comprehension in an experiment, not purchases in a supermarket. It also reflects a choice: an interpretive box that names High is milder than a stop-sign warning that says a product is high in a nutrient of concern, the approach several other countries took.
That distinction matters because the strongest population-level evidence comes from the blunter design. Chile implemented a law in 2016 that bundled black octagonal warning labels with restrictions on child-directed marketing and a ban on sales in schools of foods high in sugar, sodium or saturated fat [s3]. In a before-and-after study of 2,383 households, the volume of "high-in" beverages purchased fell 23.7 percent against a counterfactual built from pre-regulation trends (95% CI −23.8 to −23.7) [s3]. The decline was larger among more-educated households, at 28.7 percent, than among less-educated ones, at 21.5 percent [s3].
The authors are careful about what that does and does not show. The study is observational and cannot establish causality, and because Chile's policy was a package, it cannot separate the effect of the labels from the marketing restrictions or the school ban [s3]. Nor can it tell whether purchases fell because shoppers changed their minds or because manufacturers reformulated products to shed a warning [s3]. What it does show is a large, socially broad shift in buying that followed the law — and that it exceeded reductions seen from stand-alone sugar-sweetened-beverage taxes elsewhere in Latin America [s3].
What it means for a reader
The honest reading is that front-of-package labeling can move purchases, but the American proposal is a gentler instrument than the one behind the best results. Whether a Low/Med/High box changes behavior the way a warning octagon did is untested; FDA's evidence so far is that people understand the box, not that they buy differently because of it. The same reformulation question that clouds Chile's data cuts the other way here, too — if manufacturers cut sodium or added sugars to avoid a "High," the label could improve the food supply even if no shopper ever changes a decision.
For context on the nutrients the box targets, the case for cutting population salt intake rests on a large evidence base, and the debate over whether the problem is specific nutrients or processing itself is unresolved.
What to watch
Whether FDA finalizes the interpretive box as proposed or moves toward a stronger warning format after comments, whether industry challenges the mandate, and — if it takes effect — whether reformulation, rather than shopper behavior, turns out to be the main channel through which it changes what is on the shelf.
