Does vitamin D help marathon recovery? A small trial found a signal, not an answer.
Twenty-one amateur runners took vitamin D3 or a placebo before a marathon. Some muscle and oxidative-stress markers looked better in the supplement group — but most measures showed no difference.
Vitamin D has become a fixture of endurance-athlete supplement stacks, marketed for everything from bone health to immune support to exercise recovery. A small, placebo-controlled trial published this month tested one of those claims directly — whether eight weeks of vitamin D3 supplementation changes how a runner's body responds to the strain of a full marathon [s1].
What the trial did
Researchers recruited 21 amateur runners and randomly assigned them to either eight weeks of vitamin D3 supplementation or a placebo, in a single-blind design [s1]. All participants then completed an official 42.195-kilometer marathon, and the researchers drew blood at multiple points — from 24 hours before the race to 24 hours afterward — to track markers of cardiac stress, muscle damage, oxidative stress, and immune function [s1].
What moved, and what didn't
As expected, finishing a marathon produced measurable physiological stress in both groups: markers of muscle damage and cardiac stress rose after the race regardless of which group a runner was in [s1]. Among the specific markers tracked, one showed a clear group difference: protein carbonyls, a marker of oxidative stress, showed a significantly smaller post-race rise in the vitamin D3 group compared with placebo [s1]. Two markers of muscle damage — creatine kinase and its cardiac-specific fraction, CK-MB — were lower at the 24-hour mark in the vitamin D3 group as well [s1].
Other measures showed no such difference. A separate oxidative stress marker (TBARS) and antioxidant enzyme levels showed no significant interaction between supplementation and time [s1]. Both groups showed the expected post-race immune response — a rise in white blood cells, particularly neutrophils, alongside a drop in lymphocyte counts in the hours immediately after finishing — and while the study describes "time-dependent alterations" in specific immune cell subsets, it found no significant interaction effect there either, meaning vitamin D3 did not clearly change the immune trajectory compared with placebo [s1].
Reading a small, mixed result honestly
This is a trial of 21 people, split across two arms — small enough that a single outlier runner could meaningfully shift a result, and the study itself describes some of its findings as "descriptive differences" rather than statistically confirmed ones [s1]. Of the battery of biomarkers tracked, only a handful showed a statistically significant benefit tied to supplementation, while others — including most of the immune measures — showed none [s1]. That pattern is consistent with a real but modest and narrow effect on specific biochemical stress markers, not a demonstrated recovery benefit that a runner would necessarily notice in how they feel or perform.
What it doesn't show
The study did not measure whether vitamin D3 supplementation changed how runners actually felt, how quickly they returned to training, or any longer-term outcome — it tracked blood biomarkers over a 48-hour window around a single race. It also didn't report participants' baseline vitamin D status, which matters because supplementation research in people who are already vitamin D-replete tends to show smaller effects than in those starting out deficient. For a supplement now marketed broadly to runners as a recovery aid, this trial offers a narrow, cautious data point in its favor — attenuated markers of oxidative stress and muscle damage — alongside several measures where no benefit showed up at all.
