NAD+ drips are a $250-to-1,500 wellness habit with no completed outcome trials

A systematic review of 113 studies found real biology in NAD+ supplementation — and not one eligible human trial testing the IV or IM version clinics actually sell for anti-aging.

Walk into an IV-therapy storefront in Miami, Los Angeles, New York, or Dallas this month and NAD+ will be on the menu, priced anywhere from about $250 for a low-dose infusion to $1,500 for a high-dose session, with clinics marketing regular use at $8,000 to $18,000 a year [s3]. The pitch is cellular energy, slowed aging, sharper cognition. A systematic review published earlier this year, and a fresh mechanistic review that appeared August 21 in one of nutrition science's most prestigious journals, together let us check that pitch against what has actually been tested [s1][s2].

The review the industry can point to

The Annual Review of Nutrition, out August 21, is a genuine synthesis of where the science stands: NAD+ (nicotinamide adenine dinucleotide) is a coenzyme central to how cells produce energy and repair themselves, its levels decline with age, and diminished NAD+ has been implicated as a contributor to several recognized "hallmarks of aging" [s2]. The review frames NAD+ metabolism as a plausible unifying link between nutrition, metabolic resilience, and healthy aging [s2]. That is a real and current scientific position — but it is a review of mechanisms, not a report of a clinical trial, and it does not claim to establish that any particular NAD+-boosting product changes a health outcome in people [s2].

The review that checked the human trials

For that, the more useful document is a PRISMA-guided systematic review published in Ageing Research Reviews, which set out specifically to catalogue what has been tested in humans and animals between January 2010 and October 2025 [s1]. The authors identified 113 eligible studies: 80 in rodents and 33 human intervention studies, of which 28 were randomized and 5 nonrandomized [s1].

In rodents, NAD+ augmentation was frequently associated with improvements in metabolic, mitochondrial, inflammatory, and functional measures, though effects varied by model and endpoint [s1]. In humans, oral nicotinamide riboside (NR) and nicotinamide mononucleotide (NMN) — the pill-form precursors sold as supplements — consistently raised NAD+-related biomarkers in blood or cells and were generally well tolerated over weeks to months of use [s1]. But their effects on the outcomes that would actually justify a health claim — function, metabolic markers, vascular measures, and other healthspan-relevant endpoints — were "heterogeneous and often null or endpoint-specific" [s1].

Then comes the finding that matters most for the drip-bar business model specifically: no eligible outcomes trial evaluated intravenous or intramuscular NAD+ itself for anti-aging or wellness indications [s1]. One nonrandomized intravenous NMN study met the review's inclusion criteria, and it contributed only short-term safety and biomarker data, not evidence of a health benefit [s1]. A separate intravenous NAD+ pharmacokinetic pilot was included only as background context because it had no eligible clinical outcomes at all [s1].

Put plainly: the delivery method that costs the most money and requires a clinic visit is the one with the least human outcome evidence behind it. The authors' own conclusion is that NAD+ augmentation "shows clear biological activity, but clinical effectiveness for anti-aging or wellness outcomes remains inconclusive," and they call specifically for larger, well-designed randomized trials with longer follow-up "particularly for parenteral approaches" — parenteral meaning injected or infused, not swallowed [s1].

Why the gap exists

Nothing in either review suggests NAD+ IV therapy is dangerous at the doses typically marketed; the safety data that exists is reassuring, as far as it goes [s1]. The gap is about proof of benefit, not proof of harm, and it exists for an ordinary reason: an IV or intramuscular NAD+ product administered in a wellness clinic setting has not gone through the kind of randomized, outcome-focused trial that a drug seeking approval would require, because nothing about the current regulatory or business structure of the wellness-IV industry requires one. Oral NR and NMN have accumulated more human trial data simply because they are easier and cheaper to study as dietary supplements — not because they have been shown to work better [s1].

What a reader is actually being sold

A NAD+ infusion is being sold on the strength of the mechanistic story the August 21 review lays out — that NAD+ decline is linked to aging biology — combined with the biomarker changes documented for the oral, not injected, form [s1][s2]. Neither piece of evidence is a demonstrated outcome for the injected product itself. That distinction, between "there is a coenzyme involved in aging that this product changes" and "this product has been shown to make you age better," is exactly the one the systematic review found missing from the trial record [s1].

What to watch

Whether any of the ongoing intravenous NAD+ trials the review authors call for actually launch and report results — the review flags the absence, but a landmark journal publishing a mechanistic case for NAD+ in the same month tends to accelerate industry and academic interest in finally running one [s1][s2]. Readers considering NAD+ IV therapy should weigh the cost against the specific finding here: the clinical outcome trial for the injected product does not yet exist.
