The MIND diet slowed decline in cohort studies. The randomised trial found no difference.

A diet built to protect the brain looked powerful when researchers watched who chose to eat it. When they assigned it in a three-year trial, both groups improved by almost exactly the same amount.

The MIND diet — a hybrid of the Mediterranean and DASH diets, tuned toward foods linked to brain health — became one of the most recommended eating patterns for protecting cognition. Then it was put to a randomised trial, and over three years it produced no cognitive advantage over a comparison diet. The gap between those two results is one of the clearest case studies in why "this diet protects your brain" is a harder claim to prove than it looks.

Where the reputation came from

The MIND diet's evidence began, as most diet-and-brain evidence does, with observational cohorts. The researchers who devised the diet scored the eating patterns of 960 older participants in the Memory and Aging Project and tracked their cognition over an average of 4.7 years [s2]. Higher MIND scores were associated with slower cognitive decline (β = 0.0092; P < .0001), and the effect was striking in translation: being in the top third of MIND-diet adherence versus the bottom third was equivalent to being about 7.5 years younger in cognitive terms [s2].

That is the number that built the reputation. But the same paper was careful about what it could and could not show, noting explicitly that replication in a dietary intervention trial would be required to confirm the diet's relevance to brain health [s2]. Observational scores like these cannot rule out that people who eat well differ from those who do not in dozens of other ways, or that the earliest cognitive changes alter what people choose to eat.

The trial the authors called for

That trial was run, and published in the New England Journal of Medicine. It randomised 604 older adults — cognitively unimpaired but with a family history of dementia, a body-mass index over 25, and a suboptimal baseline diet — to either the MIND diet or a control diet, each with mild caloric restriction, for three years [s1]. Both groups received counselling and weight-loss support, and 93.4% completed the trial [s1].

The result was a null. Global cognition improved in both groups: by 0.205 standardised units in the MIND-diet group and 0.170 in the control group, a difference of 0.035 units with a 95% confidence interval running from −0.022 to 0.092 (P = 0.23) [s1]. Brain MRI measures — white-matter hyperintensities, hippocampal volume, total grey and white matter — changed similarly in the two groups [s1]. The diet designed to protect cognition did not measurably outperform the comparison diet on cognition.

How to read a null like this

A null trial does not mean diet is irrelevant to the brain, and there are honest caveats in both directions. The control group was not junk food: it was a defined diet with weight-loss support, so the trial compared a good diet against another reasonably good diet, which sets a high bar and could mask a real benefit over a genuinely poor diet. Three years may be too short to reveal effects that build over decades, and both groups improving slightly is partly the practice effect of taking the same cognitive tests repeatedly. The trial also enrolled people already motivated enough to change their diet.

But the caveats cut the other way too. This was a large, well-conducted, multi-year randomised trial in exactly the population the diet is marketed to, and it did not find the advantage the observational data predicted. When a strong cohort signal evaporates under randomisation, the most common explanation is that the original association was inflated by confounding and reverse causation — the same forces that trouble nearly every lifestyle-and-brain finding.

What holds up

The defensible statement is narrower than the headlines: a Mediterranean-style eating pattern is well supported for cardiovascular and metabolic health, and those benefits are worth having on their own. Its specific promise to slow cognitive decline, tested directly, did not deliver a measurable edge over another decent diet in three years [s1]. Eating well remains a reasonable thing to do for many reasons; expecting a particular "brain diet" to buy down cognitive decline is a claim the best trial evidence does not currently support.
