Menstrual cups have real safety evidence. Period underwear mostly does not yet.

For the newer reusable products — period underwear and the rest of the category — the published research is much thinner, and consists mostly of acceptability and feasibility work rather than the safety studies a shopper might assume exist [s2].
What is actually known about cups
The strongest single source is a systematic review and meta-analysis in The Lancet Public Health, which searched the literature to May 2019 and identified 436 records, of which 43 studies covering 3,319 participants were eligible for analysis [s1]. That breadth is the good news; the caveat is quality. Of the 22 studies that entered qualitative or quantitative synthesis, only three were rated moderate-to-high quality — so even the best-studied reusable product rests on a literature the reviewers themselves judged mostly weak [s1].
On the questions users care about, the findings were reassuring. In four studies that directly compared cups with usual products, menstrual blood leakage was similar or lower with the cup than with disposable pads or tampons [s1]. Across 13 studies, a pooled 73% of participants wished to continue using the cup at the end of the study (95% confidence interval 59 to 84) — though the reviewers flag high statistical heterogeneity in that figure [s1]. Four studies covering 507 women found no adverse effects on vaginal flora [s1].
The safety review did surface rare harms, and reporting them honestly matters more than a blanket reassurance. Across the whole literature the reviewers identified five women who reported severe pain or vaginal wounds, six reports of allergies or rashes, nine of urinary-tract complaints (three of them with hydronephrosis, a swelling of the kidney), and five cases of toxic shock syndrome after cup use [s1]. They also found 13 women in whom an intrauterine device was dislodged during cup use [s1]. Set against the millions of cup users worldwide — the review counted 199 brands available across 99 countries — these are uncommon events, and the review's overall interpretation was that cups "are a safe option" while good-quality studies remain needed [s1].
Where period underwear and the newer products sit
Period underwear, absorbent reusable briefs and similar products are marketed with the same sustainability and convenience pitch as cups, but the evidence base is not comparable. The published studies that exist are largely about whether people would use and accept such products, not clinical safety trials. A representative example is a 2018 qualitative study that explored the "hypothetical acceptability and potential utility" of reusable menstrual underwear among 30 refugee women, with input from five humanitarian staff, in a settlement in Greece [s2]. Its output was themes about comfort, appearance, absorbent capacity and hygiene — useful for product design, but a long way from a safety evaluation [s2].
That gap is the answer to the everyday question. When a person asks whether period underwear is "safe" or "healthy," the honest reply is that there is little published clinical evidence either way — not that harms have been found, and not that safety has been demonstrated. The absence of evidence is not evidence of a problem, but it is also not the reassurance that a well-studied product like the cup can point to.
The distinction worth keeping
Two things are true at once. The reusable-product category as a whole is plausible and popular, and there is no body of trial data flagging it as dangerous. But the depth of evidence varies sharply within it: the menstrual cup has a systematic review concluding it is safe, with the rare adverse events catalogued; period underwear and most newer entrants have acceptability research and little else [s1] [s2]. A marketing claim that a product is "clinically proven" or "doctor-tested" is doing work the published literature, for most of these products, does not yet support.

Most breast pain is self-limited, and cancer is rarely the cause — though it should be ruled out. A 2025 meta-analysis found no clear benefit from vitamin B6, a widely recommended remedy.
PMS and PMDD are not the same thing, and only one has strong drug evidence
PMDD is the severe, mostly psychological form of premenstrual illness, affecting about 3.6% of women in one study. For it, SSRIs are the best-evidenced treatment, with side effects that are common and dose-related.
