Does collagen powder do anything for your skin? The trials say a modest yes

Pooled randomised trials show real improvements in skin hydration and elasticity versus placebo, but the effect is moderate, the measures are instrument readings, and the evidence base is shaky.

Oral collagen supplements do produce measurable improvements in skin hydration and elasticity against placebo when randomised trials are pooled, so the category is not pure marketing — but the effect is moderate, the gains are mostly instrument readings rather than how skin looks in the mirror, and the underlying studies are small, short and methodologically weak [s1][s2][s3]. The most defensible reading is that collagen powder probably does a little, in the short term, for objectively measured skin properties, and that the confidence the advertising projects is not matched by the quality of the evidence.

What the pooled trials found

A 2025 systematic review and meta-analysis in the Indian Journal of Dermatology, Venereology and Leprology combined ten randomised controlled trials with 646 participants, using studies published between 2010 and 2021 [s1]. Collagen supplements significantly increased skin hydration (standardised mean difference 1.25, 95% CI 0.77 to 1.74) and elasticity (SMD 0.61, 95% CI 0.21 to 1.02) compared with placebo [s1]. The most commonly used dose was 4 g/day, with a median of 3.5 g/day across the trials [s1].

An SMD above one, as for hydration, is a large effect in statistical terms; the elasticity effect is moderate. But the review is candid about what sits underneath those figures: heterogeneity was moderate (I² of 55.5 to 56.5%), the risk of bias in the included studies was generally unclear, and publication bias could not even be assessed because there were too few studies [s1]. "Could not be assessed" is not the same as "ruled out" — in a field dominated by manufacturer-run trials, the studies that found nothing are the ones most likely to be missing.

A consistent, and consistently limited, literature

The finding is not a one-off. An earlier and larger meta-analysis in the International Journal of Dermatology pooled 19 studies with 1,125 participants and reported that hydrolyzed collagen improved hydration, elasticity and wrinkles versus placebo, concluding that 90 days of supplementation reduced signs of skin ageing [s2]. A 2026 review in Frontiers in Medicine analysed 19 randomised trials of oral and topical skin-ageing peptides in 1,341 participants, again finding that oral formulations improved hydration and brightness [s2][s3].

That review is the most useful for calibrating expectations, because it put a number on the cosmetic outcome people actually care about. The pooled effect on wrinkle reduction was statistically significant but modest (mean difference 0.27, p = 0.04), with the benefit driven largely by oral polypeptides in subgroup analysis (mean difference 1.5, p = 0.01); effects on elasticity and skin density were inconsistent [s3]. On the reassuring side, the peptides were well tolerated, with minimal adverse events reported across the trials [s3].

The caveats that matter

Three limitations run through all three reviews and should shape how the numbers are read. First, the participants are overwhelmingly women — 95% in the International Journal of Dermatology analysis — so the findings say little about anyone else [s2]. Second, the outcomes are objective instrument measurements of skin hydration and elasticity, which is a strength for reproducibility but does not automatically translate into a visible difference a person would notice. Third, and most important, the evidence quality is low: unclear risk of bias, unassessable publication bias, and trials that are short and small [s1].

Where a household claim rests heavily on trials funded by the companies selling the product, that provenance is part of the story. The reviews here pool those trials without being able to correct for the missing negative ones [s1], which means the true average effect is plausibly smaller than the pooled figure suggests — the same caution that applies to most of the supplement shelf.

How it fits the wider picture

Collagen is sold for skin, joints and "anti-ageing" broadly, and the quality of the evidence drops as the claims widen. For skin, there is at least a consistent signal on hydration and elasticity [s1][s2]. That is a better evidence base than many products carry — stronger, for instance, than the case for oral supplements aimed at joint pain, and a different kind of intervention from the at-home light devices marketed for the skin. It is also not a biological-age test or a longevity intervention, whatever the packaging implies.

What to watch

The question the field has not answered is whether collagen's instrument-measured gains mean anything a person sees or keeps. That needs larger, longer, independently funded trials with patient-rated outcomes and enough studies to test for publication bias — exactly what the current reviews say is missing [s1][s3]. Until then, the honest claim is narrow: a modest, short-term effect on measured skin hydration and elasticity, in mostly female participants, from a literature that cannot yet rule out that it is overstating its own results.
