Kerala confirms another Nipah virus case — the district's sixth known outbreak since 2018

A single patient is on ventilatory support in Kozhikode. WHO says 104 contacts are being monitored and the source of infection has not been identified.

Kerala's Kozhikode district has recorded its sixth confirmed Nipah virus outbreak since the state's first in 2018. On 11 June, the Kerala State Health Department confirmed a laboratory-positive case in an adult male resident, who had developed symptoms on 30 May and was hospitalized on 10 June [s1]. As of WHO's most recent update, he remains on ventilatory support in an intensive care unit [s1].

What is known about the case

The patient's presentation was primarily neurological, without reported respiratory symptoms before he was intubated [s1]. Initial positive results came from PCR testing at local laboratories and were subsequently confirmed by RT-PCR at the National Institute of Virology in Pune, India's reference laboratory for the virus [s1].

Contact tracing began immediately after confirmation. As of 18 June, 104 contacts had been identified: four classified as very high-risk, 14 high-risk, and 86 low-risk, including 45 health and care workers [s1]. All are under active monitoring with regular follow-up, and WHO reports no secondary cases to date [s1].

Why one case triggers this level of response

Nipah virus has no approved vaccine or specific antiviral treatment, and reported case fatality in past outbreaks has been high. Human infections range from asymptomatic to acute respiratory illness to fatal encephalitis, with an incubation period WHO lists as typically 3 to 14 days, though incubation as long as 45 days has been documented in rare cases [s1]. Person-to-person transmission through close contact is established, which is why a single confirmed case triggers immediate, wide-radius contact tracing rather than a wait-and-see approach.

Fruit bats of the genus Pteropus — flying foxes — are the natural reservoir. Transmission to humans occurs through contact with infected animals, or through fruit or fruit products contaminated with bat saliva, urine or excreta, including raw date palm sap in South Asia [s1]. WHO's report is explicit that in this case, "the source of infection has not yet been identified," and that "given the known presence of animal reservoirs, additional cases cannot be excluded" [s1].

Part of a recurring pattern

This is not Kozhikode's first encounter with the virus. WHO's report notes the event follows "a pattern of recurrent Nipah virus outbreaks in Kerala, including in Kozhikode district, since the first outbreak was reported in 2018," with additional outbreaks in 2019, 2021, 2023, 2025 and now 2026, according to India's National Centre for Disease Control [s1].

That recurrence is a feature of the region's ecology rather than an anomaly. Kerala sits within the range of Pteropus bat populations, and repeated spillover events in the same district over eight years point to an ongoing, low-frequency transmission risk rather than a one-time event. What distinguishes each outbreak is less whether spillover will happen again and more how quickly it is detected and contained once it does.

What is different, and what is not

So far, this event involves a single confirmed case with no identified secondary transmission [s1] — a more contained picture than some past Kerala outbreaks, which have included clusters linked to hospital exposure. The 45 health and care workers among the current contact list reflect standard precaution around a patient who was hospitalized and treated before his diagnosis was confirmed, not evidence that transmission has already occurred among them.

What to watch

Whether any of the 104 identified contacts, particularly the four classified very high-risk, develop symptoms within the virus's incubation window. Whether Kerala's ongoing investigation identifies the source of this patient's exposure — unresolved as of WHO's latest report — which would help clarify whether the case reflects a specific point exposure or a broader circulation of the virus in the local bat population. And whether contact monitoring extends the full incubation period, including the extended-incubation cases WHO's own guidance acknowledges as possible, before the event is considered closed.
