WHO traces a global infant formula contamination to one Chinese oil supplier

Products reached 99 countries before recalls began. WHO's first Disease Outbreak News report on the event lists 144 suspected and confirmed illnesses and calls the risk moderate.

The World Health Organization issued its first Disease Outbreak News report on a global infant formula contamination event on 13 March, three months after the first product recalls began [s1]. The report puts a number on the scale of the problem: formula, other nutritional products, and the oil ingredient behind the contamination reached 99 countries and territories across six WHO regions [s1].

What was contaminated, and with what

The contamination traces to arachidonic acid (ARA) oil, an ingredient added to infant formula as a source of omega-6 fatty acid, intended to approximate a component of breast milk [s1]. The oil was sourced from a single supplier in China [s1]. Testing of it turned up cereulide, a heat-stable toxin produced by certain strains of the bacterium Bacillus cereus that survives the pasteurization and processing steps that would normally kill the bacteria that make it.

Cereulide intoxication generally causes gastrointestinal symptoms — nausea and vomiting — that in most people resolve without lasting harm [s1]. Infants are a higher-risk population specifically because vomiting-driven fluid loss can progress to dehydration and electrolyte disturbance more quickly in a small body than in an older child or adult [s1].

The first product recalls began 10 December 2025, well before this WHO report [s1]. WHO's overall risk assessment is "moderate" [s1] — a rating driven less by the severity of individual illnesses, which are generally mild, than by three compounding factors the report lays out explicitly: the population affected is infants, among the most vulnerable groups to dehydration; the full scope of distribution and exposure remains uncertain; and gaps persist in both case detection and the root-cause investigation itself [s1].

Where the cases have been reported

Of the 144 suspected and confirmed cases, the largest national totals came from the United Kingdom (61) and Spain (41), with smaller counts in France (11), Austria (9), Belgium (8), Brazil (5), Czechia (4), Singapore (3), and single cases in China/Hong Kong and Italy; Denmark and the Netherlands also logged self-reported cases [s1]. Belgium is notable as the only country in the report with laboratory-confirmed, rather than suspected, cases [s1] — a distinction WHO's report attributes to the practical difficulty of confirming cereulide intoxication through testing, since the toxin's gastrointestinal symptoms overlap with many other common causes of infant vomiting and there is no routine clinical test most health systems run by default.

Why the geographic spread matters

Ninety-nine countries and territories is an unusually wide distribution for a single-supplier contamination event, and it says something about how consolidated the global infant formula supply chain has become. A single ingredient supplier feeding into products sold under multiple international brands means a localized manufacturing problem can surface as a genuinely global public health event rather than a national or regional one — which is part of why this reached the level of a WHO Disease Outbreak News report rather than being handled purely through national food-safety recall systems.

The report does not name the specific formula brands involved, focusing instead on the shared oil ingredient as the root cause — a framing that puts the emphasis on supply-chain traceability for parents and health authorities rather than on any single manufacturer's product line.

What to watch

Two threads from WHO's own report point to what comes next. First, the root-cause and traceability investigation is explicitly unfinished, meaning the current case count and country list could still grow as more batches are traced and more health systems report illnesses retrospectively linked to the contamination [s1]. Second, the moderate risk rating is conditional on those same uncertainties resolving — a firmer picture of how widely contaminated batches actually reached consumers, versus how much of the 99-country distribution involved product that was recalled before reaching infants, would let WHO either raise or lower that assessment with more confidence. Parents in affected countries are directed to national recall notices and health authority guidance for product-specific information, which — unlike the epidemiological overview in WHO's report — changes as individual countries complete their own batch tracing.
