More than 1,800 dead across four countries, and the health emergency is only starting

Floods and landslides in Indonesia, Sri Lanka and Thailand have killed more people in a fortnight than most single climate events on record. The second wave — water, food, medicine — is the part that lasts.

The counting is not finished, which is itself part of the story.

By 7 December, floods and landslides across Sumatra, Sri Lanka and Thailand had killed more than 1,800 people [s3]. Sri Lanka's confirmed toll stood at 618 with 209 still missing, 464 of the deaths in the tea-growing central highlands [s3]. Indonesia had passed 900 [s3]. Thailand had recorded at least 276 [s3]. Malaysia and Vietnam had two deaths each [s2][s3].

Five days earlier, the same regional figure had been 1,160 [s1]. The gap between those two numbers is not new flooding. It is bodies being reached.

The scale underneath the deaths

Mortality is the number that travels. It is rarely the number that determines the health burden.

More than two million people in Sri Lanka — close to a tenth of the population — were affected by the floods and landslides triggered by Cyclone Ditwah [s2]. More than 75,000 homes were damaged and about 5,000 destroyed [s3]. At its peak, 225,000 people were in emergency accommodation; by 7 December, around 100,000 remained [s3].

In Indonesia, more than 500,000 people were displaced across North Sumatra, West Sumatra and Aceh [s1]. In Thailand, 3.9 million people and 1.5 million households were affected [s1].

Those displacement figures are the health story. A population moved out of its homes, into crowded temporary accommodation, without reliable clean water, is the standard precondition for the diseases that follow floods.

The second wave

The immediate causes of death in a flood are drowning, crush injury and landslide burial. The deaths that accumulate afterwards come from water, food and interrupted care.

Reporting from Aceh Tamiang in Indonesia described residents drinking contaminated water while waiting for aid, with children falling ill as a result, and people walking an hour across debris to reach government assistance [s3]. Officials in flood-hit parts of Indonesia reported shortages of food, shelter and medicine [s2].

Aceh's governor, Muzakir Manaf, warned that starvation was among the gravest threats to remote villages, saying people were dying not from the flood but from the absence of food afterwards [s2].

That progression — from acute trauma, to waterborne illness, to malnutrition and untreated chronic disease — is the predictable shape of a flood emergency. It is also the phase in which the count becomes least reliable, because deaths from diarrhoeal illness or an unmanaged chronic condition in a displacement camp are rarely attributed to the disaster that produced them.

What attribution can and cannot say here

It is tempting to reach straight for a climate figure. The honest position in early December is narrower.

Indonesia's president, Prabowo Subianto, said after visiting North Sumatra that the country needed to confront climate change effectively [s1]. Sri Lanka's president, Anura Kumara Dissanayake, declared a state of emergency and described the flooding as the largest and most challenging natural disaster in the country's history [s1]. Al Jazeera's reporting identified illegal logging in Sumatra alongside climate change as intensifying the disaster [s2] — and Indonesia's forestry minister moved to revoke logging licences covering 750,000 hectares held by 20 companies, while the environment minister halted palm oil, mining and power plant operations upstream of the affected areas [s2].

No rapid attribution study assigning a share of this rainfall to warming had been published as of this writing. What exists is the general finding, set out in the 2025 Lancet Countdown report in October, that extreme weather and its health consequences are worsening as warming continues, and that the burden falls disproportionately on populations with the least capacity to absorb it [s4].

Land use and rainfall are separable causes in principle and inseparable in a specific catchment. Both are being cited by the governments involved [s2].

What to watch

Three things over the coming weeks. Whether disease surveillance is stood up in the displacement sites, and whether it publishes. Whether Sri Lanka's request for international assistance is met — the country was already fiscally constrained, and its president said the economy was not strong enough to absorb the shock alone [s3]; the IMF was considering an additional $200 million [s3]. And whether the landslide risk in Sri Lanka's central highlands, still feared as of 7 December [s3], produces a further wave of casualties.

The reconstruction estimate for Sri Lanka alone runs to $7 billion [s3]. Health system recovery is a line item inside that, and it is usually the slowest one.
