FDA clears Ilumya for active psoriatic arthritis, its second indication
The IL-23 inhibitor tildrakizumab, until now a psoriasis-only drug, won approval on 29 October for active psoriatic arthritis in adults. Two phase 3 trials met their main mark, by modest margins.
The Food and Drug Administration has given tildrakizumab a second job. On 29 October 2026 the agency approved the antibody, sold by Sun Pharma as Ilumya, for active psoriatic arthritis in adults — adding a joint disease to a drug that until now was licensed only for moderate-to-severe plaque psoriasis of the skin [s1]. The clearance was recorded under biologics licence application 761067, supplement S-031, and the revised label is dated October 2026 [s1][s2].
Tildrakizumab is an interleukin-23 antagonist: it blocks IL-23, a signalling protein that drives the inflammatory cascade behind both psoriatic skin plaques and the joint inflammation of psoriatic arthritis. The two conditions often travel together, which is part of the logic for testing a psoriasis drug against the arthritis. The new label now lists both indications side by side — plaque psoriasis in adults and active psoriatic arthritis in adults [s2].
What the trials showed
The approval rests on two multicentre, randomised, double-blind, placebo-controlled phase 3 trials, together enrolling 806 adults with active psoriatic arthritis who had at least three swollen and three tender joints [s2]. The trials differed deliberately in the patients they took in: Trial PsA2 enrolled people who had already been treated with a tumour-necrosis-factor (TNF) inhibitor, while Trial PsA3 enrolled patients who had never had one [s2][s3][s4]. In both, patients received Ilumya 100 mg by subcutaneous injection at week 0 and every 12 weeks, or placebo for 24 weeks [s2].
The primary endpoint was the share of patients reaching an ACR20 response at week 24 — a 20 per cent improvement across a composite of joint and symptom measures. In the TNF-experienced trial, 41 per cent on Ilumya hit ACR20 versus 25 per cent on placebo, a difference of 15 percentage points [s2]. In the TNF-naive trial, 35 per cent reached it versus 23 per cent, a 12-point gap [s2]. Both differences were statistically significant.
It is worth reading those numbers honestly. ACR20 is a relatively low bar, and roughly a quarter of placebo patients cleared it too. On the sterner measures the picture was mixed: the 50 per cent response (ACR50) separated from placebo in the experienced group but not clearly in the naive group, and the 70 per cent response (ACR70) reached significance only in the naive trial [s2]. This is a real but moderate treatment effect, not a dramatic one.
How it is dosed
Dosing depends on the patient. For adults who have active psoriatic arthritis together with moderate-to-severe plaque psoriasis, the regimen is the same as for psoriasis: 100 mg at weeks 0 and 4, then every 12 weeks [s2]. For other adults with psoriatic arthritis, the label drops the loading dose and recommends 100 mg every 12 weeks [s2]. Either way, it is a four-times-a-year injection given by a healthcare provider — an infrequent schedule that is part of the drug's appeal in a crowded field of psoriatic-arthritis biologics.
Where it fits
Psoriatic arthritis already has several approved biologic options, including TNF inhibitors, IL-17 blockers and other IL-23 agents. Tildrakizumab enters as another IL-23 choice, and its trials were built to show it works whether or not a patient has failed a TNF drug first [s3][s4]. That design matters: patients who have exhausted a TNF inhibitor are often the hardest to treat, and a drug that holds up in that group as well as in treatment-naive patients is more useful to prescribers weighing where it belongs. The label's standard precautions carry over from the psoriasis approval, including the requirement to evaluate patients for tuberculosis before starting, because dampening this arm of the immune system can let latent infection surface [s2].
What to watch
The near-term question is where tildrakizumab lands in treatment sequences — whether its every-12-week schedule and the mixed ACR50/ACR70 results earn it a place ahead of, or behind, the alternatives. Longer-term safety in joint disease will accrue as use widens.
Red flags: anyone on an immune-modulating biologic who develops a persistent fever, night sweats, a worsening cough or other signs of infection should be assessed promptly, as these drugs can blunt the body's response to infection. This article describes an FDA approval and is not medical advice. Choices about biologic therapy for psoriatic arthritis belong with a rheumatologist or dermatologist who knows the individual case.
